  1. Switch the student's Primary Residence from Parent(s) to Extended Family.

  2. Scroll down until you see the additional Parent/Guardian contact field.

  3. Clear out all information listed in that additional contact form, including the state abbreviation.

  4. Scroll back up and switch the Primary Residence back from Extended Family to Parent(s). You should now be able to sign the form, and your signature should be accepted.
