This step-by-step guide will help you select a sport, complete the necessary medical forms, and print the physical form for your student.

To begin, you'll need to log in to your FinalForms account. Once you're logged in, follow these simple steps:

First, locate and click on the blue Incomplete Forms button. This will take you to your student's sports selection page. Please note that the physical and medical forms will populate only after a sport is selected.

After selecting a sport, you will need to fill out the Health History and Medical Profile forms. Once you have completed these forms, you will see the final PPE Physical (for Physicians) form with a Print button. This form will automatically populate with the information you provided in the Health History and Medical Profile forms.

Once you've completed your student's medical forms, navigate to your My Students page. Here, click the + PPE Physical button for your student. This action will generate a PDF of the completed PPE Physical form, which you can take to your physician for review and signature.
