How to Complete the Participation Survey

Welcome to the Participation Survey Guide! At FinalForms, we are dedicated to making the collection of annual participation data as seamless as possible. Whether your school is a FinalForms customer or utilizing our free NFHS Participation Survey site, we've got you covered with a straightforward process and plenty of support.

Steps to Complete Your Participation Survey

  1. Go to https://search.finalforms.com.

  2. Search for your school.

  3. Click the Login button under the Staff icon.

  4. Click the Calendar icon.

  5. Select the current school year.

  6. Navigate to Manage > Schools.

    Note: The Participation Survey must be completed for every member school in your district. District-level staff may submit for all schools, or individual school administrators may submit for their own building.

  7. Locate the member school you need to submit.

  8. Click the black Export button in the row for that school.

  9. Select the State Participation Survey.

  10. Review your school's sports and activities offerings on the Verify Sports & Activities page. Add any official or unofficial sports your school offers here to populate the correct offerings for the Participation Survey.

  11. Review your School Enrollment Numbers (these come from previously existing data).

  12. Update the numbers based on your current enrollment.

  13. Click Verify Counts.

Roster-Based Counts

For FinalForms Customers

If your school is a FinalForms customer, you'll enjoy the convenience of having your rosters automatically pre-populate participation numbers in your survey. All you need to do is review the information, verify its accuracy, and submit!

Participation numbers for FinalForms customers come directly from your FinalForms rosters. To correct them:

  • Click the sport name.

  • View or modify the roster as needed.

  • Finalize your changes.

  • Return to the survey and continue.

For Non-FinalForms Customers

For those who are not FinalForms customers, any rosters you previously uploaded will pre-populate when possible. If no roster data is available, you will need to enter participation numbers in the survey manually.

For non-customers, numbers may appear as zero if you have never assigned students to rosters for previous reporting. In this case, you will need to manually enter or correct all participation counts.

Submitting Your Survey

  1. Review all numbers for accuracy.

  2. Click Submit to send your data to your state association.

Re-Submission Policy

You can submit and re-submit your Participation Survey at any time before your state's deadline. Rest assured, the most recent submission will be used by your state association.
