Accessing the Annual Checklist Form
To begin editing the Annual Checklist form, please follow the instructions outlined below:

Navigate to the Annual Checklist via the Calendar Icon located on the top navigation bar.
Under the Current School Year Annual Checklist (i.e., 2025-26 Annual Checklist), click on the Schools option.
Select the specific high school you wish to edit.
Editing the OHSAA Tournament Participation Form
Once you have accessed the high school section, you will see the OHSAA Tournament Participation form on the far left-hand side of the screen. Follow these steps to make the necessary changes:
Click on the OHSAA Tournament Participation form.
Scroll down to locate the specific sport you need to change for the tournament.
Make the required changes to the form.
After making the changes, continue to scroll down to the signature section.
Sign your name and submit the form to complete the process.
