How to Resolve PowerSchool 404/500 Error

When you see the 404/500 error codes, the contact has an ID assigned to them in FinalForms that belongs to a record in PowerSchool that no longer exists:
Clicking the Change Match button to change to a different contact will remove that old contact ID and resolve the issue.
